About the Role
As a provider at Matrix, you will conduct thorough health assessments during visits that can last up to one hour. You will review each member's medical history, medication adherence, and social environment to gain comprehensive insights into their health and well-being. Key responsibilities include :
- Performing adult and geriatric assessments, including medical history, diagnosis, treatment, health education, and case management referrals.
- Administering and ordering point of care and lab-based testing as necessary.
- Collaborating with physicians, case managers, social workers, and family members to ensure coordinated care.
- Providing services at the member's residence, whether at home or in Skilled Nursing Facilities.

Master's Degree or equivalent experience with NP or PA licensure.Current NP or PA licensure in the state of practice with prescriptive authority.Board certification in relevant specialties.Current BLS, ACLS, or CPR certification.Preferred 1 year of experience as an NP or PA; new graduates are encouraged to apply.Valid driver's license and adequate automobile insurance.Strong computer skills and proficiency with medical record software.Excellent verbal and written communication skills.The Matrix Culture
