About the Role

As a provider at Matrix, you will conduct thorough health assessments during visits that can last up to one hour. You will review each member's medical history, medication adherence, and social environment to gain comprehensive insights into their health and well-being. Key responsibilities include :

• Performing adult and geriatric assessments, including medical history, diagnosis, treatment, health education, and case management referrals.
• Administering and ordering point of care and lab-based testing as necessary.
• Collaborating with physicians, case managers, social workers, and family members to ensure coordinated care.
• Providing services at the member's residence, whether at home or in Skilled Nursing Facilities.
