Your Role :

  • Provide compassionate direct patient care as per the state Nurse Practice Act.
  • Follow through on the care plan created by the registered nurse.
  • Document patient care and activities accurately and promptly.
  • Educate patients and families on self-care techniques and relevant health information.
  • Coordinate home health services and communicate patient needs to other healthcare professionals.
  • Use and manage medical equipment and supplies, following best practices for safety.
  • Engage in personal and professional development opportunities.
  • Perform additional duties as assigned by the registered nurse.
  • Deliver health care instructions to patients based on assessments and care plans.

    • Qualifications :

  • Graduate from an accredited practical nurse or vocational nursing program.
  • A minimum of three years of nursing experience, preferably in community / home health or medical / surgical settings.
  • Current licensing as a licensed practical nurse or licensed vocational nurse in the state.
  • Valid CPR certification required.
  • Must have reliable transportation, a valid driver's license, and appropriate automobile insurance.
