Job description

The Caregiver Support Program serves as a CSP RN Clinical Assessor responsible for conducting standardized assessments to support eligibility decisions for the Program of Comprehensive Assistance for Family Caregivers PCAFC. Applies expert knowledge of program criteria, nursing process, and VISN operations. The mission of the CSP promote caregiver health and well-being through collaboration, education, and support, ensuring high-quality care for Veterans and their families.

Note : Applicants selected from the Eugene, OR area will be required to travel to Roseburg, OR to complete mandatory training.

The Caregiver Support Program (CSP) Registered Nurse (RN) Clinical Assessor executes position responsibilities that demonstrate leadership, experience, and creative approaches to the management of complex patient care. Duties include but not limited to :

  • Provides care and counseling of persons in the promotion and maintenance of health and prevention of illness and injury based upon the nursing process which includes systematic data gathering, assessment, appropriate nursing judgment, and evaluation of human responses to actual or potential health problems through such services as case finding, health teaching, and health counseling.
  • Possesses thorough knowledge of PCAFC, clinical documentation requirements, current standards of care, and all regulatory compliance requirements and guidelines.
  • Accountable for utilizing critical thinking skills and sound judgment to ensure caregivers are provided with a thorough, comprehensive review when appropriate.
  • The CSP Clinical Assessor will develop and collaborate with the interdisciplinary team for case review with high inter-rater reliability, efficiency and objectivity as top priorities.
  • Demonstrates knowledge and ability to apply principles of the CSP and CEAT practice.
  • Applies the nursing process and care coordination principles within a collaborative, interdisciplinary practice setting, which will then enable effective application of CSP practices and principles
  • Uses knowledge of the CSP to matrix Veteran level of functioning with the appropriate CSP services (i.e., Program of General Caregiver Support Services [PGCSS]; PCAFC; and / or Veteran entitlements and benefits) in order to ensure patient safety, recovery, and program stewardship.
  • Evaluates, systematically, current practice and formulates outcomes for groups of patients and / or organizational processes within area of expertise.
  • Coordinates within the CSP to develop and implement strategies for performance improvement and enhancement of the program.
  • Supports colleagues and other nurses through knowledge sharing to provide safe, quality nursing care. Fosters a safe and supportive environment conducive to the professional development of health care professionals.
  • Evaluates outcomes of evidence-based decisions and practice changes for individuals, groups, and populations.
  • Assists the VISN with MISSION Act implementation, as it relates to CSP and PCAFC expansion. Coordinates within the CSP to develop and implement strategies for performance improvement and enhancement of the program.
  • May coordinate with other Regional and Veterans Affairs Central Office (VACO) CSP entities to develop and implement strategies for PCAFC eligibility, tier level, discharge, and appeals processes and performance improvement nationally.
