License : Must have active nursing license

Must-Have : Proficiency in HomeCare HomeBase (HCHB) and OASIS

Job Description :

• Provide, coordinate, and direct home nursing care according to physicians' orders, following agency policies and procedures.
• Supervise and evaluate Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses.
• Conduct home visits and participate in periodic office meetings.
• Must have reliable transportation, a current driver's license, and required liability insurance.
• Manage an average of 5-8 patients per day, equating to 6-8 productivity points.
• Perform various types of cases such as Start of Care, Wound Care, and Follow-up Visits.
• Travel up to 75 miles from the home clinic, with mileage reimbursed at the IRS standard rate.
• Complete all documentation within 24 hours of the patient appointment.
• Be open to on-call rotations and flexible scheduling, which may include some weekends depending on the location.
• Ensure all patient appointments are accepted regardless of distance to maintain service standards.

Additional Information : This role is specifically for Home Health assignments, with weekly meetings required. Candidates must possess a vehicle, driver's license, and auto insurance and must be able to meet the weekly productivity targets by accepting all assigned patients.
