We are currently looking for a Full Time Women's Care / Obstetrics Unit RN to join our OB Team located at the Chickasaw Nation Medical Center.
Summary :
Plans and provides routine patient care by utilizing and documenting the nursing process.
Responsibilities :
- Coordinates the transportation of patients to other healthcare facilities and / or a higher level of care at CNMC when necessary
- Adheres to departmental and hospital safety practices, policies and procedures, and unit practices based upon industry standards / requirements
- Supports the Baby Friendly Initiative 10 steps and Infant Feeding policy and procedure
- Documents nursing care accurately, completely, and legibly
- Collaborates effectively with other disciplines as appropriate
- Transfers care as appropriate utilizing the SBAR system
- Plans and provides comprehensive patient care by utilizing and documenting the nursing process : including teaching patients and their families formulating and evaluating the nursing care plan, and discharge planning
- Provides the patient and / or significant other(s) with education including the safe and effective use of medications, diet and nutrition, safe and effective use of medical equipment, and pain management
- Recognizes and anticipates common changes in the physical, biopsychosocial, and health education needs of female adult and female adolescent patient populations experiencing pregnancy related and non-pregnancy related conditions, acute medical and surgical gynecological conditions, post-partum conditions, and labor and delivery conditions
- Recognizes and anticipates common changes in the physical, biopsychosocial, and health education needs of the neonate in both stable condition and those experiencing acute medical condition
- Administers prescribed medications, IV fluids, and blood products according to facility policies and procedures
- Administers special medications for OB according to facility policies and procedures including Magnesium Sulfate, Cytotec, and Pitocin
- Provides and advocates age and culturally appropriate, holistic patient care based upon facility mission statements and as outlined in policies and procedures
- Participates in hospital, departmental, and unit performance improvement activities
- Assesses the patient on admission and reassesses as indicated, including pain assessments
- Delivers quality patient care in accordance with the OB / GYN / NURSERY Standards of Care
- Documents assessment, interventions, all aspects of patient care, and patient's response to interventions according to protocol
- Provides a conducive learning atmosphere and clinical experience for students
- Executes the appropriate line of support when faced with decisions
- Implements emergency procedures and / or interventions related to the age and size of the patient
- Provides individualized, goal directed nursing care to well and sick neonates through use of assessment, planning, implementation, and evaluation from admission to discharge
- Monitors hemodynamic status of the patients and initiates emergency protocols appropriate for the age and condition of the patient
- Monitors fetal heart tones and communicates to provider appropriate findings utilizing NICHD criteria
- Identifies problems competently and provides interventions according to established protocols
- Provides independent and dependent interventions to achieve and maintain optimal patient responses, restore stability, and prevent complications of the newborn
- Assesses the need for restraint initiation, provides alternatives to restraints, monitors for physical psychological signs of distress related to restraint, document use of restraints, and discontinues restraints as appropriate
- Provides a safe environment for patients, staff, and visitors by the reduction and control of environmental hazards and risks and the prevention of accidents and injuries
- Completes appropriate Universal Protocol for patients undergoing procedures and initiates the "Time Out"
- Assures complete documentation and that the surgical consent and H&P are signed, witnessed, and on the chart
- Assures that all ordered diagnostic tests are performed and the results are posted in the medical record
- Performs recovery room nursing duties for the immediate post-op patient
- Acts as a patient advocate and maintains confidentiality, security, and integrity of data and information
- Functions as a circulating nurse as assigned by leadership
- Performs aseptic techniques in preparing for and assisting with surgical procedures
- Calls breaks and / or omissions in technique to the attention of the operating staff and take measures to correct the situation
- Assures access to the operative area, and uses precautions to avoid obstruction of circulation, respirations, and protection of nerves from compromise of undue pressure
- Prepares operative area by cleansing skin
- Assists in maintaining surgical record including procedure, time, participants, time-out, blood loss, medications, positioning, etc
- Prepares and labels specimens for pathological examination and monitoring of sponge, needle, and instrument counts
- Ensures the proper working order of special / surgical equipment
- Observes the need for supplies during operation and replacing them as necessary
- Functions as an OB charge nurse as assigned by leadership
- Makes appropriate and safe patient assignments while facilitating the flow of admissions, discharges and transfers
- Oversees OB triage and provides assistance as needed
- Communicates with House Supervisor and staff nurses about patient status changes
- Supplements staff in a clinical role as needed to ensure the continuum of care
- Ensures that staff provide safe and effective patient-centered care throughout the care areas by assimilating evidence-based standards into decisions
- Utilizes problem-solving skills and provides direction, support, and constructive feedback to the OB Nursing Team members throughout each shift
- Acts as a clinical resource for OB Nursing personnel using current literature and guidelines, organizational policies and procedures, and regulatory and accreditation standards
- Ensures that all OB Nursing staff function according to these same requirements
- Documents nursing care, incidents, accidents / injuries, and other issues in an accurate, complete, and legible manner
- Reviews the induction schedule as well as staffing for the current and upcoming shifts and notifies OB Nurse Manager and House Supervisor regarding concerns or special needs as necessary
- Shares responsibility for and verifies the general upkeep of the OB department, including the availability of appropriate equipment and supplies and notifies the OB Nurse Manager of any unmet needs
