Description :

This position involves providing therapy services to patients in their own homes.

The patient population mainly consists of geriatric individuals.

You will conduct comprehensive assessments and evaluations, and develop treatment and care plans.

Oversight of Physical Therapist Assistants will be part of your responsibilities.

You will work with other healthcare professionals including nursing and social work staff.

Typical types of visits are weighted differently on a point system.

Candidates with some knowledge of the new payment model coming in January 2020 will be preferred.

The ideal candidate is easy-going, enjoys meeting people, and is accepting and friendly.

Business casual attire is required; closed-toed shoes must be worn.

Weekends may be required as needed but are not a routine expectation.
