Job description

A company is looking for a Triage Care Manager.

Key Responsibilities

Assist Participants in coordinating care for Long Term Services & Supports and closing care gaps

Conduct assessments to identify barriers for care coordination and implement appropriate interventions

Collaborate with various healthcare providers to effectively coordinate care and make referrals as necessary

Required Qualifications, Training, and Education

Bachelor's Degree in Nursing or a health-related field preferred

Current and unrestricted Registered Nurse licensure in Pennsylvania or an Unrestricted Compact License

Social Worker with an unrestricted Pennsylvania clinical license

Minimum of 3 years of nursing experience in a related clinical setting with Discharge Planning

Managed Care experience required; case management experience preferred
