Care Navigator

A company is looking for a Care Navigator for the East Coast. Key Responsibilities Process referrals to secondary care and maintain directories of specialists and community services Manage communication and documentation for referrals to ensure continuity of care Serve as the primary contact between patients, specialists, and clinic physicians regarding external care needs Required Qualifications At least 3 years of applicable experience, preferably in a Primary Care setting High school diploma or GED required Proficient in Google spreadsheets and / or Excel, with experience in database management and data tracking
