The role you'll contribute :
The Admissions Nurse is responsible for responding to and visiting with patients who have been referred for hospice consultation and or admission to AdventHealth Hospice Care. The Admissions Nurse assesses the appropriateness / qualifications of the patient for Hospice services and informs the patients, caregivers, and family members of the hospice services available with the organization. In addition, the Admissions Nurse is responsible for visiting current AdventHealth Hospice Care patients who have been admitted to an AdventHealth hospital.
The value you'll bring to the team :
Provides information and available resources to patients / family / caregiver for admittance onto Hospice services.
Provides referrals / support for follow-up care for patients who are uninsured or underinsured.
Travels to the patients location or hospital room of patient who has been referred to Hospice.
Partners with the hospital Case Management staff member to meet the needs of the patients and hospital staff to gain new referrals.
Assesses the patient and familys emotional condition as to readiness for hospice. Answers questions and responds to concerns to encourage admission onto the Hospice program if conditions are justified.
Coordinates efforts for patient treatment and promote quality improvement and educational efforts.
Obtains required signatures and generates electronic and or paper documents as required to comply with regulations for reimbursement.
The expertise and experiences you'll need to succeed :
Minimum qualifications :
Associates degree
3 years Registered Nurse experience in home health and / or acute care nursing experience; including 1-year experience in the hospital
Registered Nurse State Licensure and / or Compact State Licensure
Active American Heart Association BLS
Valid in state Drivers License with current car insurance
Preferred qualifications :
Bachelor of Science in Nursing
Certified Hospice and Palliative Nursing certification.
