Responsibilities

Perform diagnostic assessments and gather essential clinical information to create individualized, evidence-based treatment plans tailored to each client's unique needs.

Offer one-on-one and group counseling sessions focused on problem-solving, emotional support, and connecting clients with vital community resources.

Work alongside physicians, nurses, social workers, and other care team members to develop and refine care strategies that promote recovery and well-being.

Continuously evaluate client progress with follow-up assessments, refining treatment goals to reflect clinical findings and changing needs.

Be a calm and confident presence during moments of crisis, providing immediate intervention, clear guidance, and stabilization strategies.

Qualifications

Education :

Requires a high school diploma or GED and a minimum of five years documented clinical experience in mental health or human services; or

An individual possessing a certificate of psychiatric rehabilitation from a DHS-approved program, plus a high school diploma or GED, plus two years' documented experience in providing mental health services or

An individual possessing a bachelor's degree in counseling and guidance, rehabilitation counseling, social work, education, vocational counseling, Psychology, pastoral counseling, family therapy, or a related human service field.

License(s) / Certification(s) :

Valid driver's license.

(CPI) training within 6 months of hire.

Obtain certification within 2 months of hire and re-certify annually according to the requirements set forth in the Illinois Department of Healthcare and Family Services related to the Illinois Medicaid Comprehensive Needs & Strengths (IM+CANS)
