Schedule : This is an in person position Monday through Friday 8 : 00AM - 5 : 00PM.

Wage Range : $68500.00 - $80000.00 salaried Exempt $1500 sign-on bonus

Position Summary :

The Health Related Social Needs ( HRSN) Manager supports the delivery of Whole Health services by managing day-to-day operations related to the Social Determinants of Health (SDOH) across NARA NW programs. This position provides support and guidance to staff and clients helps facilitate access to community resources and works collaboratively with internal teams and external partners to ensure high-quality services to families elders and children / youth. The Manager implements culturally appropriate practices and contributes to program improvement efforts.

What you will do :

Complete timely approvals of HRSN (Health-Related Social Needs) program referrals to ensure appropriate service delivery.

Supervise HRSN staff providing support guidance and performance feedback.

Support accurate and timely documentation across various platforms and systems.

Track grant spending and reporting details to ensure compliance and fiscal accountability.

Approve grant-related purchasing and manage documentation for audit readiness.

Track and ensure completion of required staff trainings related to job functions compliance and cultural competency.

Establish and maintain positive supportive relationships with clients and families to provide encouragement and culturally sensitive services.

Conduct outreach and in-person engagement to assess client needs related to Social Determinants of Health (SDOH) and assist in developing care or service plans.

Serve as an advocate and liaison for clients and families with external community agencies and partners.

Maintain working knowledge of and provide referrals to community services and resources including Aging and Disability Services Housing Assistance Transportation Food / Shelter Services Medicaid LIHEAP / OEAP and Public Health resources.

Provide culturally informed support to staff working directly with clients to ensure culturally responsive service delivery.

Monitor client progress track key service metrics and report findings to support quality improvement efforts.

Collaborate with Program Managers and clinical teams to coordinate client services across departments.

Participate in staff training and development sharing information on resources best practices and culturally specific approaches.

Attend internal and external meetings representing Social Services and sharing relevant updates with staff and leadership.

Promote a Whole Health approach that incorporates recovery physical and behavioral health and spiritual well-being.

Assist with leading team huddles and program meetings to support communication and program alignment

Qualifications

We would like to hear from people that have :

Bachelors degree in social work public health human services or a related field; OR at least 4 years of equivalent experience working with complex populations and High school diploma or GED.

Minimum of 2 years of direct experience working in social services healthcare behavioral health or a related setting.

Minimum of 1 year of supervisory experience

Demonstrated ability to work effectively with underserved and high-risk populations including individuals from diverse cultural and socioeconomic backgrounds.

Strong communication and interpersonal skills with the ability to build relationships with clients staff and community partners.

Knowledge of local and federal community resources and experience helping clients navigate service systems.

Experience working within Native American / Alaska Native communities preferred.

Valid drivers license and proof of insurance.

Ability to maintain professional boundaries and handle sensitive information with confidentiality.

Must pass a pre-employment or for cause drug tests.

Must pass criminal background and DMV checks.
