Requirements :

Active New York RN license

Nurse Compact / Multi-state license (required)

Minimum 3+ years of diabetic care experience

Experience in care coordination or case management (preferred)

Must provide own technology (computer, webcam, microphone, speakers, phone, stable internet)

Strong communication, critical thinking, and organizational skills

Responsibilities :

Provide remote case management for diabetic patients

Conduct virtual assessments and ongoing monitoring

Develop, implement, and modify individualized care plans

Collaborate with multidisciplinary healthcare teams

Document all patient interactions per facility standards

Ensure compliance with state regulations and organizational policies

Maintain confidentiality and secure handling of patient data
