Job Description

We are seeking a dedicated Case Manager RN to join our team. The ideal candidate will have a strong background in acute hospital case management, utilizing their skills in discharge planning and utilization management. You will play a vital role in ensuring quality patient care and facilitating a smooth transition for patients as they move through the care continuum.

Responsibilities

• Conduct utilization reviews and ensure compliance with CMS guidelines.
• Develop and implement discharge plans for patients across diverse age groups, including geriatric, adult, adolescent, women, and infants.
• Work collaboratively with the healthcare team to optimize patient care.
• Utilize InterQual criteria in the case management process.
