Physical Therapist - Outpatient Care (FT, PT or Per diem)

FT, 80 hrs / pp, 1.0 FTE; days, varies as scheduled.

PT, 40 hrs / pp, 0.5 FTE; days, varies as scheduled.

Per diem / on call; varies as scheduled.

Job Summary :

The staff licensed physical therapist evaluates, examines, treats, monitors and provides ongoing assessment of patients / clients; performs all necessary paperwork / reports; plans and develops new programs; maintains communication with patients / clients, patient / client family, caregiver and physician and contract site staff; presents in-services; assists in general management of the department; supervises therapy assistants and support staff; and other diverse duties as requested or required.

Education and / or Experience Requirements :

Bachelor’s (B.A., B.S.), Master's (M.A., M.S.) or Doctorate (DPT) degree in Physical Therapy

Licensure & Certifications Requirements :

Physical Therapy board certification and licensure according to state regulations.

Supervisor's license as required by state.

Valid Driver's license.

Pay Range :  $43.00-$65.00
