Job Description
Administer skilled nursing care for clients of all ages in their residence.
Coordinate care with the interdisciplinary team, patient / family, and referring agency.
Assess home health patients to identify physical, psychosocial, and environmental needs.
Implement, develop, and evaluate the Plan of Care (POC) to ensure quality care.
Educate patients and families regarding care, diet, medications, and home safety.
Supervise and provide clinical directions to support staff to ensure continuity of service.
Provide effective communication and serve as a liaison between patients, families, and healthcare professionals.

Education / Licenses Needed
Current state license as a Registered Nurse is required.
Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program is required.
CHPN certification preferred.
Proof of current CPR certification is required.
