As an active member of the medical community in your territory, you will provide both ongoing palliative care and hospice service and education. Qualifications : A heart to serve patients and families and a passion for providing the best possible care Education : Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing preferred Experience : 2 years experience within the last 5 years in chronic disease management / long-term care. Hospice background preferred Required : Reliable transportation.
