PT- Rehab needed asap

Performs a comprehensive Physical Therapy assessment of patients referred for service including but not limited to physiological, psychosocial, environmental factors and health history.

Bachelor of Science Degree or Master's Degree in Physical Therapy from an institution accredited by the Counsel on Medical Education of the American Medical Association and by the American Physical Therapy Association.

Required Qualifications

Physical Therapist, Home Health

Experience : 1 year

Licenses : PT-MA

Certifications : BLS-AHA(Copy needed)

SSN Required

DOB Required

Work History : Should not be more than 30 days of gap in entire work history

Bachelor of Science Degree or Master's Degree in Physical Therapy from an institution accredited by the Counsel on Medical Education of the American Medical Association and by the American Physical Therapy
