Applicant must have own transportation.  Will primarily work in Mason County WV, but will be, as needed, required to travel to Cabell, Wayne, Lincoln County in WV, or Lawrence County Ohio.

We take care of patients in their homes, assisted living, personal care homes, nursing homes, or wherever they call home.

Duties :

• Performs and / or assists the patient with personal care activities as prescribed on the Home Health Aide plan of care.
• Performs and / or assists the patient / family with light housekeeping and meal preparation.
• Reports patient status changes to the interdisciplinary team as needed.
• Maintains current and accurate documentation.
• Complies with policies and procedures of Hospice of Huntington, Inc.
• Protects the confidentiality of private health care information of patients.
• Represents the services and philosophy of Hospice of Huntington, Inc.
