The Registered Nurse, CareLine, is responsible for providing telephonic emergency triage and directional patient care advice for general and specific illnesses, health related issues, client counseling, patient advocacy, health education and referral and resource management to ChenMed patients and their families. Providing on-call coverage, the incumbent in this role provides remote clinical advice and assessments within license and as possible given the technology and medium. He / She collaborates with primary caregivers and others on the interdisciplinary care plan team to provide a team approach of care.
