Job description

SUMMARY

The Licensed Alcohol and Drug Counselor (LDAC) or Licensed Independent Clinical Social Worker (LICSW) is responsible for diagnosing substance use disorders and providing treatment, education, and support to individuals and families. This position promotes healthy lifestyles and substance use disorder recovery through evidence-based practices and collaborative care. The LDAC or LICSW provides direct counseling services, case management, and appropriate referrals to ensure clients receive comprehensive, person-centered care. This position is located in Littleton, NH

REPORTS TO : Service line director

SUPERVISES : None

QUALIFICATIONS

  • Bachelor's or Master's degree in Counseling, Psychology, Social Work, or a related field from an accredited institution.
  • Current New Hampshire LDAC license in good standing (MLDAC preferred) or LICSW licensure.
  • Preferred Impaired Driver Care Management Program (IDCMP) credentials or willingness to receive training to attain this credential
  • Minimum of one (1) year of experience in healthcare, behavioral health, or substance use treatment setting.
  • Knowledge of co-occurring disorders and evidence-based treatment modalities such as Motivational Interviewing, Cognitive Behavioral Therapy (CBT), and relapse prevention.
  • Excellent interpersonal, organizational, and communication skills.
  • Experience using Electronic Health Record (EHR) systems preferred.
  • Ability to maintain positive working relationships with staff, community partners, and clients at all levels.

ESSENTIAL FUNCTIONS

  • Provide individual, group, and family counseling to individuals experiencing substance use disorders.
  • Conduct substance use assessments, diagnostic evaluations, and treatment planning using ASAM criteria.
  • Deliver education and counseling regarding the effects of drugs and alcohol.
  • Collaborate with LRH providers and external partners to coordinate comprehensive care for patients.
  • Accept referrals and act as a liaison between patients and treatment programs.
  • Facilitate referrals to appropriate levels of care, including detoxification, residential, or outpatient treatment programs.
  • Assist patients in exploring available community resources, including housing, recovery supports, and employment services.
  • Maintain confidential, accurate, and timely documentation of all client interactions and treatment progress in accordance with agency policy and 42 CFR Part 2 and HIPAA regulations.
  • Participate in interdisciplinary case reviews and treatment team meetings to ensure quality and continuity of care.
  • Create and maintain a resource library of information on substance use, prevention, and recovery supports.
  • Participate as a member of the SBIRT (Screening, Brief Intervention, and Referral to Treatment) team.
  • Ensure that all patients are appropriately screened using approved screening tools.

  • Provide brief interventions and counseling to those identified through the SBIRT process.
  • Engage in professional development and maintain continuing education requirements for licensure.
  • Perform other related duties as assigned.

  • Apply recovery-oriented and client-centered approaches in all services.
  • Exhibit sound clinical judgment, problem-solving, and crisis-intervention skills.
  • Maintain professional boundaries and confidentiality at all times.
  • Participate in quality improvement initiatives and compliance audits as needed.
  • COMPLIANCE AND ETHICAL STANDARDS

  • Ensure all services are delivered in accordance with 42 CFR Part 2, HIPAA, and NH DHHS regulations.
  • Uphold agency policies and procedures regarding documentation, privacy, and patient rights.
  • Report any suspected violations or ethical concerns to the Service Line Director or Medical Director or Compliance Officer immediately.
  • WORK ENVIRONMENT

  • Work is performed in a clean, well-lit office environment.
  • Occasional travel to community partner sites or off-site meetings may be required.
  • May require occasional evening hours to accommodate client or program needs.
