Currently, we are looking for a Licensed Vocational Nurse (LVN) to join our Community Health team as part of the CalAIM ECM Program. In this dynamic role, you will play a crucial part in delivering essential healthcare services to underserved populations, ensuring that every individual receives the attention and support they deserve. ‭ Pay : ‬ ‭ $30.00 - $35.00 per hour‬

‭ Expected Hours : ‬ ‭ Monday through Friday 8 : 30AM to 5 : 00pm (40 hours per week)

Work Location : Hybrid remote in Santa Clara County, California

Responsibilities

• Provide direct nursing care to clients in community settings
• Assist in the assessment and evaluation of clients' health needs
• Develop and implement individualized care plans in collaboration with the healthcare team
• Monitor and document clients' health status and progress
• Educate clients and their families on healthcare, wellness, and preventive measures
• Coordinate with multidisciplinary teams to ensure a holistic approach to care
• Support clients in accessing mental health and social support services
