LOCATION : Greater LA (Downtown, ELA, South LA (Downey, Southgate, Alhambra)- Dialysis Centers, Nephrology offices, patient homes

SHIFT : 8am-5pm (M-F)- Monday–Friday with some flexibility- No nights, no weekends, no on-call

JOB TYPE : Full-Time Permanent

We work directly with local dialysis centers, nephrology offices, and patients in their homes to improve access to care, reduce avoidable hospital visits, and create a better experience for those managing long-term health conditions.

What You’ll Be Doing

• Deliver comprehensive, relationship-based care to adult and senior patients in a mix of clinic settings with an occasional in home and telehealth
• Conduct health assessments, screenings, and chronic condition management visits
• Develop and implement individualized care plans that address both clinical and lifestyle needs
• Partner with nephrologists, dialysis center staff, and care coordinators to ensure continuity of care
• Order and interpret diagnostic tests and communicate findings with patients and families
• Maintain accurate, compliant documentation in the EHR
• Participate in team discussions to drive quality outcomes and enhance the patient experience
