RN Case Manager

RN Case Manager Highlights The pay for this position is $70-$80 per hour . 4x10s schedule : 7 : 30am 6 : 00pm with weekend rotation. Onsite position. This position is a 13-week contract role. Strike coverage. Responsibilities Support physicians and the interdisciplinary team to facilitate patient care and optimize clinical outcomes, patient satisfaction, and cost efficiency. Manage utilization review, care coordination, patient education, and discharge planning for an assigned caseload. Facilitate precertification, authorization, and communication with payors. Identify and remove barriers to timely care and reimbursement across the continuum. Apply process improvement methodologies to evaluate and enhance care outcomes. Coach and support clinical documentation accuracy; serve as a resource for coders to ensure documentation reflects severity of illness. Coordinate communication with physicians and clinical teams. Demonstrate knowledge and competencies appropriate for all patient populations served. Requirements BSN required. Active RN license in the State of Michigan required. BLS certification through AHA required. Minimum 5 years of acute inpatient case management experience , including acute discharge planning. Experience with CarePort required. Preferred : Experience charting in AllScripts, GEMS / Sunrise; experience in utilization management, discharge planning, performance improvement, or managed care reimbursement. Strong communication, negotiation, analytical, and organizational skills. Ability to work independently and exercise sound judgment when interacting with physicians, payors, patients, and families.
