Your Specific Responsibilities:

This is a part time position that will work 2-3 days per week. This is a responsible nursing position in the Jefferson County Health Department (CHD) that provides a variety of community health nursing services. This position supports clinical nursing coverage. Responsibilities include, but are not limited to:

Clinic Activity: Renders skilled nursing care for all clients at the County Health Department. Responsible for all client-clinic activity including, but not limited to, the following: Adult Health, Obstetrics, Family Planning, Immunizations, Sexually Transmitted Disease, Florida Breast and Cervical Cancer Program (FBCC), Medication [Insulin and Epilepsy] Assistance Programs, Pharmacy, Laboratory, and Phlebotomy services. Operates within all clinic programs according to policies, procedures and guidelines as set forth by the Department of Health (DOH) and any other applicable regulatory bodies. Maintenance of these programs requires activities including but not limited to the following: completion and submission of program applications, adhering to programmatic requirements, monitoring and reporting client's medical status to clinician team, referral completion and follow up, specific data monitoring, and communication with team members across DOH programs.

Electronic Medical Record (EMR): Records all patient data accurately in Health Management System (HMS), assures standardization of medical record documentation.

Providers: Assists all licensed providers of the CHD including, but not limited to, triage, assessment, procedures, and examinations.

Triage: Independently assess clients' medical, behavioral, and socioeconomic needs. Refer clients to DOH programs and connect clients to resources and community partners.

Maintenance: Maintains all clinic equipment, supplies, medications, and vaccinations; this includes inventory accountability, ordering, documentation, and compliance with program standards.

Quality Assurance/Quality Improvement: Engages in quality assurance and quality improvement activities as assigned. Specifically, monitors EMR performance and corrective action via chart audits.

Phone: Maintain clinic phone service by triaging incoming calls and messages and providing follow-up to the client within 24 hours.

Training and Collaboration

Meetings: Attends and engages in meetings as they relate to clinic and operational activities. These include but are not limited to: internal staff meetings, statewide programmatic meetings, and collaborative partner meetings.

Training: Attends continuing education activities as assigned, especially as required for programmatic compliance. Training will include statewide, programmatic, and local assignments both in person and virtually. Some training will require travel.

Collaboration: Intentionally collaborates with DOH partner programs including but not limited to: Healthy Start, Tobacco Prevention, Disease Intervention Specialist, Heart Health Plus, Women Infants and Children (WIC), and Epidemiology. Implements client referrals and facilitates linkage to care to program partners.

Team Support: Provides clinical support to Madison CHD as needed. Provides clinic environment conducive to training nursing students.

Other

Employee Activities Records (EAR) and Client Service Records (CSRs): Promptly enters CSRs and time in EARs into HMS, as specified by the DOH coding manual and CHD performance requirements. Submits leave and attendance records to supervisor at minimum bi-weekly, or sooner as directed by supervisor.

Required Knowledge, Skills, and Abilities:

Cultural Competency: Deep respect for cultural and social differences to effectively communicate health information to individuals from diverse backgrounds, foster client trust, and to provide culturally competent care.

Autonomy: Ability to operate independently under clinic protocols and provider standing orders.

Clinical Skills (not limited to): Comprehensive health assessments; identification of social, behavioral, economic, and medical health risks; medication and vaccine administration, phlebotomy, procedural set up and assistance; and diagnostic testing and interpretation of results; laboratory specimen collection, preparation, and processing; operation and maintenance of obstetric, procedural, and clinic equipment.

Leadership and Collaboration: ability to work effectively with diverse professionals across various programs and community sectors.

Community Engagement and Education: Knowledge of population health issues to impactfully educate individuals and communities on healthy behaviors including reproductive health and disease prevention, wellness promotion, prenatal care, chronic disease, and social determinants of health.

Record Keeping and Documentation: proficiency in maintaining accurate, organized medical records and adhering to privacy regulations.

Time Management: Ability to prioritize tasks, manage time effectively, and operate a busy clinic environment independently and across multiple service areas.

Other job-related requirements for this position:

Incumbent may be required to work before, during and/or beyond normal work hours or days in the event of an emergency. Emergency duty required of the incumbent includes working in special needs or Red Cross shelters, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural.

This position requires travel. A valid driver's license is required. Vehicle not provided by the State.

The incumbent in this position is responsible for the integrity and security of data sets under his or her control.

Incumbent will have access to records containing social security numbers in the performance of job duties.

Quality Improvement: Identifies areas for improvement and suggest improvement projects to address these areas. Initiates problem solving processes and/or QI improvement projects in conjunction with managers and other appropriate staff. Assist Performance Management Council (PMC) in the identification of performance data needs and sources. Complies program data for performance assessments. Identifies and reports staff training needs to QI coordinator.

Qualifications:

Minimum -

Licensure in good standing as a Registered Nurse in accordance with Florida Statute 464.

Current cardiopulmonary resuscitation (CPR) certification.

Active Driver's License in accordance with the laws and regulations of the issuing state.
