Summary :
Provides primary wound care; assessing, treating, documenting, and tracking all wounds and skin treatments; responsible for assessing nail care treatment and requirements and providing routine care.
Education and Experience :
- Graduation from a state-approved nursing program.
- Current license to work as a Registered Nurse in the Commonwealth of Virginia.
- One year experience; understanding of the geriatric resident and their needs.
- Excellent communication, interpersonal, and documentation skills.
- WOCN certification is desired but not required.
Essential Functions :
Wound Care :Conduct and document a thorough assessment of each resident's skin upon admission and throughout the resident's course of treatment.Under the supervision of DON, assist the implementation of the resident's treatment plan.Comply with evaluation, treatment and documentation guidelines of Birmingham Green wound protocols.Complete required documentation in an accurate and timely manner.Provide regular resident skin status updates to appropriate personnel.Attend and participate in facility wound rounds bi-weekly or as required; attend and participate in department / facility meetings as required.Maintain ongoing communication with physicians concerning skin treatments.Clinical Nail CareResponsible for assessing Nursing Home resident's nail care / treatment requirements.Responsible for trimming all routine toenails as needed. Will communicate needs of residents who require routine fingernail care to Head Nurses and CNAs. Will be responsible for follow up.Responsible for coordinating Podiatrist treatment of residents who are insulin-dependent or receiving Coumadin (or other blood thinner) and assisting Podiatrist with care / treatment as needed. Responsible for keeping a log of such treatment.Responsible for documenting routine nail care given in resident's charts.Responsible for cleaning and maintaining nail care instruments and adhering to sanitary cleaning procedures.
