Hospice of the Midwest is seeking a compassionate grounded and spiritually-sensitive Chaplain in Woodbury MN. Our Chaplains provide emotional and spiritual support to patients and families during end-of-life care. This role is an integral part of our interdisciplinary team offering a nonjudgmental presence and care that honors the beliefs values and cultural backgrounds of those we serve. As a Chaplain You will :
Conduct and ensur e timely spiritual assessments for patients and familiesParticipate in developing individualized care plans to address spiritual needsProvides direct spiritual support to patients families and caregiversServes as a liaison to community chaplains and spiritual counselorsMaintains accurate documentation of spiritual services and communicationsAttends interdisciplinary meetings and contributes to care planningOffers education and spiritual care guidance to the care teamRecruits and supports community chaplains spiritual counselors and volunteersCoordinates and leads memorial or funeral services as requestedOrganizes periodic memorials for staff volunteers and community partnersAssists in supervising spiritual care volunteersQualifications
Graduate of an accredited theology or chaplaincy program (BD MDiv or equivalent theological degree)Experience working with death and dying hospice experience preferredComfort in an interfaith settingMust be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working orderAbility to accept different lifestyles cultures beliefs and valuesKnowledge of and commitment to hospice philosophy of careAbility to work as member of interdisciplinary groupAbility to network with community clergy and congregationsAbout our Line of Business
