Outreach Worker

The Center for Human Development, (CHD) is seeking a Bilingual Outreach Worker for our Adult Community Clinical Services (ACCS), located in Springfield, MA. ACCS is funded through the Department of Mental Health and is a multidisciplinary outreach approach for persons served in need of psychiatric and other services living independently in the community. This involves active engagement including assertive outreach, consistent assessment and treatment planning, skill building, symptom management, addiction treatment, and behavioral and physical health monitoring and support.

YOUR ROLE AS DIRECT CARE :

ACCS teams include licensed clinicians, RNs, housing specialists, substance use specialists, direct care staff, and peer / family specialists. The Team works collaboratively to provide person-centered, evidence-based services in support of Persons' treatment goals. The Direct Care Staff assists participants in need of psychiatric services acquire the skills necessary to live independently in the community. S / he works within the Agency's team model, establishing open communication, support, and accountability among co-workers.

REQUIRMENTS :

• At least one year experience working with population preferred
• Bachelor's degree from an accredited educational institution preferred (required for DC II, or additional experience)
• High School diploma required
• Interested candidates should be Bilingual (English / Spanish). With ability to speak, read and write in both fluently

ESSENTIAL JOB FUNCTIONS :

Pay Rate : $20 / hr
