Job Requirements

Education Requirements : Graduate from a Physical Therapist program
Certifications Requirements : WI PT License, AHA BLS, Any required certifications
Experience Requirements : Minimum of 1 year of recent experience in a hospital setting
Other Requirements : Valid Driver's License, Last 4 of Social Security Number, Ability to float to inpatient MS units as needed, Flexibility with shift hours and schedule, Strong charting skills with Epic experience preferred

Job Responsibilities

Provide specialized physical therapy assessments and develop individualized treatment plans for patients in both outpatient and inpatient settings.

Collaborate with interdisciplinary teams to ensure coordinated patient care and optimal recovery outcomes.

Document patient progress, treatment interventions, and relevant clinical information accurately in Epic and other electronic medical records.

Manage patient caseloads efficiently, including scheduling and float responsibilities to inpatient medical-surgical units as needed.

Educate patients and their families on treatment plans, post-discharge exercises, and health maintenance strategies to promote long-term wellness.
