A company is looking for a Patient Navigator to facilitate onboarding and engagement for Managed Medicare Risk Members. Key Responsibilities Initiate contact with new members and provide an overview of available services and benefits Document interactions and outcomes in Epic, ensuring accurate member data and attribution Collaborate with care coordinators to ensure timely member outreach and follow-up Required Qualifications High School Diploma / GED or higher 2+ years of experience in analyzing and solving customer problems 2+ years of experience in the healthcare industry Intermediate computer proficiency, including Microsoft Outlook, Excel, and Word Access to a designated quiet workspace and high-speed internet connection
