APC | Diabetes Education & Management

Mercy Health in Defiance, Ohio, is seeking a compassionate and dedicated Advanced Practice Provider (Family Nurse Practitioner or Physician Assistant) to join our Diabetic Education team. This role is central to helping patients living with Type 1 and Type 2 diabetes gain confidence, control, and clarity in managing their health.

About the Opportunity

In this position, youll empower patients through education, individualized care plans, and consistent support as they navigate chronic disease management. You will serve as both a clinical provider and a trusted partner in each patients long-term health journey.

This role includes travel among three local Mercy Health practice sites in the Defiance and Napoleon areas, ensuring high-quality diabetes education and care is accessible throughout the region.

New graduates with a passion for diabetes care are encouraged to apply, and applicants with one to two years of experience in endocrinology, diabetes management, or a related specialty are strongly preferred.

Position Highlights

Outpatient-focused schedule : Monday-Friday, 8 : 00 AM-5 : 00 PM

No traditional call occasional PerfectServe notifications for urgent needs

Telehealth services including virtual visits and e-visits

Typical patient volume : 15-22 patients per day

Established patient base, including absorption of a departing providers panel

Strong clinical support, including onsite CMA assistance

Travel to three practice locations within Defiance and Napoleon

Clinical Responsibilities

Conduct comprehensive health assessments and detailed patient histories

Develop individualized diabetes education plans and treatment strategies

Guide patients in lifestyle modification, glucose monitoring, medication adherence, and long-term self-management

Order and interpret diagnostic studies as appropriate

Collaborate with physicians, educators, and multidisciplinary care teams

Manage MyChart messages, patient calls, and specialty referrals

Provide both onsite and telehealth patient care

Engage in professional duties consistent with APC practice standards
