Registered Nurse

We are seeking Registered Nurses to join our Team! You will be responsible for the assessment management and treatment of assigned patients.

Responsibilities Include :

• Visit clients in the privacy of their residence in designated geographic territories.
• Schedule and preform monthly supervisory visits
• Schedule and preform Quarterly Evaluations
• Complete timely and accurate clinical notes including addressing patients progress.
• Evaluate the client status and the skills of workers while the staff member is present.
• Keeps documentation of all communication with clients family members and support staff. .
• Provides patient family and / or caregiver education and information pertinent to diagnosis and safety issues.
• Counsels and educates the patient and family in meeting healthcare and related needs.
• Write 485 / Plan of Care for patients when reauthorizations are due for patients.
• Other projects / duties as assigned.
