This position offers a Monday through Friday schedule from 7 : 30 AM to 4 : 00 PM, with flexibility available. The patient volume is low, averaging 912 patients per day, primarily focused on managing chronic conditions related to diabetes, heart, and kidney health. The care team includes an MD, APP, and MA.
