Cleveland Area Hospital is an independent, self-managed Critical Access Hospital half an hour west of Tulsa, Oklahoma. Our facilities include a level IV Trauma Center / ER, Skilled Nursing / Swing Bed program, a fully equipped Outpatient Rehabilitation Center, 24 / 7 Lab, and a Primary Care Clinic. POSITION SUMMARY : The physical therapist will organize and implement specific treatment procedures / protocols for individual clients according to the principles and practices of physical therapy. The therapist will collaborate with the interdisciplinary treatment team. The therapist will provide services including treatment, planning, implementation, education, documentation, and communication for the purpose of assisting the clients to achieve their goals, optimal functional level, and improved quality of life in a multitude of settings.

RELATIONSHIPS : The physical therapist reports to the Rehab Services Director and services customers to include patients, patients' families, physicians, therapy department employees, and hospital employees.

EDUCATIONAL / SKILLS REQUIRED :

The physical therapist must be a graduate of an accredited physical therapy school, successfully complete the National Certification Examination for Physical therapist, possess a current OK Physical Therapist license or be eligible via temporary license, and qualify for malpractice coverage.

EXPERIENCE :

More than one year is preferred.
