Quality Outreach Coordinator

A company is looking for a Quality Outreach Coordinator to conduct clinical member outreach and assist in closing care gaps.

Key Responsibilities

Conducts incoming and outgoing calls to educate members on closing gaps in care

Maintains accurate records of member interactions and issues

Acts as a liaison with providers and assists members with scheduling appointments

Required Qualifications

Associates degree in healthcare or 3 years of relevant work experience

2 years of experience in case management or community health

Minimum of 2 years experience working with members to close care gaps

At least 1 year of managed care experience

Knowledge of managed care or health insurance industry and medical terminology
