Title : Care Coordinator

Location : Nassau County (Hybrid – 2 days in-office, Hempstead, 3 days of home visits / field work across Nassau County)

Schedule : M-F 9-5 (2 days in office, 3 days in field)

24.72 hourly, $45k annually | Full-Time, 35 hour work week |

About the Role

Our client is seeking bilingual (Spanish-speaking) Care Coordinators to join their interdisciplinary team. You'll work primarily in the field, managing a caseload of 40–50 adults and / or children with Serious Mental Illness (SMI), co-occurring substance use disorders, and complex medical needs. This role blends in-person outreach and telephonic support to help clients achieve greater independence and reduce hospitalizations.

Key Responsibilities

• Conduct initial and ongoing client assessments to develop individualized service plans
• Act as a liaison to link clients with medical, behavioral, residential, and entitlement services
• Deliver crisis intervention and support as needed
• Maintain accurate and timely documentation of client progress
• Educate clients and team members on conditions, interventions, and available community resources
