Texas and Colorado Licensed Clinical Navigator

A company is looking for a PT Clinical Navigator responsible for delivering patient-centered care navigation for individuals managing musculoskeletal conditions.

Key Responsibilities

Conduct clinical intake assessments and evaluate patients' musculoskeletal conditions

Provide education and guidance to help patients understand treatment options and adhere to care plans

Monitor patient progress and address barriers to care through personalized strategies

Required Qualifications

Doctor of Physical Therapy (DPT) or Master of Physical Therapy (MPT) required

Current, unrestricted PT licenses in Texas and Colorado

Minimum 2 years of relevant experience in physical therapy or MSK-related roles

Willingness to obtain additional state licensure as needed

High comfort level with technology, including Google Suite and CRMs
