IV skills and wound care experience required. Experience with general medical diagnoses and high-risk diagnoses such as heart failure, COPD, myocardial infarction, pneumonia, and stroke. Must have recent patient care experience in the last 3 months. Must have valid driver's license and vehicle for home visits. Must be COVID vaccinated or have an approved exemption form on file. Flu shot or declination is required, and mandatory mask wearing must be observed while at work. Description : The Home Health Registered Nurse will be responsible for providing skilled nursing care to patients in their homes. The nurse will perform cardiopulmonary assessments and basic wound care. This position requires management of compression dressings, wound vac dressings, and Pleurx / Asept drain care. The nurse will perform Foley catheter changes and care, staple / suture removal, and utilize glucometer and pro-time machines. Administration of subcutaneous and intramuscular injections, lab draws, and venipuncture will also be required. The nurse will perform IV skills, including PICC line or Midline care and dressing changes, as well as subcutaneous port access and deaccess. The role includes conducting home skilled visits, with an expected range of 6-8 visits per day. Scheduling of visits and hours may vary based on the needs of the home care office. There may be alternating weekend shifts involved. The nurse will use a laptop and MiFi for internet access and portable printers for documentation. All documentation must adhere to established systems and standards. Interdisciplinary support will be available from physical therapy, interpretation services, social services, and pharmacy. Candidates must be adaptable to changing patient needs and work collaboratively with healthcare professionals. A smoke-free environment is maintained.
