Job Description

American Traveler is seeking an experienced Home Health Physical Therapist for elderly patient care, requiring NJ licensure and BLS certification.

Responsibilities

Home health setting providing care to elderly patients in Mercer County and North Burlington County

This role is part of the PACE-LIFE (Living Independence for the Elderly) program

Work schedule is Monday through Friday, 8 : 00 AM to 4 : 30 PM

No on-call or weekend shifts required

Regular home visits are required for patient care

Responsibilities include conducting evaluations, developing and implementing individualized treatment plans, and providing therapy to improve mobility and function

The role involves educating patients and caregivers on exercises and home-based care strategies

You will collaborate closely with physicians, nurses, and case managers as part of a dedicated care team

Maintaining accurate and timely documentation is an important component of this position

No weekend or on-call work is required, supporting a work-life balance

Requirements

Graduate of an accredited Physical Therapy program

Valid NJ Physical Therapist license required if applicable

AHA BLS certification required

Minimum 2 years of recent experience with adult and geriatric populations

Experience in home health and senior care is essential

Strong communication, documentation, and problem-solving skills are required

Must have reliable transportation

Frequent standing, walking, and driving required

Ability to occasionally lift up to 50 lbs., bend, and reach

Experience in pelvic floor therapy preferred

Training in Herman & Wallace Level 1A or equivalent preferred
