Overview

To support the Mission of Helio Health, Inc., the CTI Peer Case Manager will provide comprehensive care management and coordination of services to Critical Time Intervention (CTI) program enrollees. The CTI model is a time-limited form of case management that focuses on helping people with severe mental illness (SMI) and a history of homelessness during a "critical time" transitional period in their lives. CTI is designed specifically to enhance the continuity of care during transition from hospital to community care.

CTI Peer Case Manager :

Helio Health is on a mission to treat and promote recovery from the effects of substance use, mental health disorders, and other behavioral healthcare issues. To support the mission of Helio Health, the Critical Time Intervention Peer Case Manager acts as a care manager with lived experience, but in addition to Care Management tasks, they resolve medical and rehabilitative needs, when within scope of practice, that impact the individual's ability to meet recovery goals. The Peer Case Manager will work on a Full-Time basis providing all services in the community and using a hybrid work model when not in the community.

The schedule is Monday-Friday, 8 : 00am-4 : 30pm, with one late shift per week (12pm-8 : 30pm), and some evening on call responsibilities.

Pay Rate : $23.00-$24.52 per hour

In addition to Helio Health's comprehensive benefits package, Full-Time hires for this position are eligible for a Sign-On Incentive between $1,500- $3,000 depending on licensure!

Responsibilities

All client visits are expected to be completed in the community, meeting people where they are at.

Carry a primary caseload providing CTI services.

Develop person-centered care plan.

Share knowledge of and provide linkage to resources.

Engage individuals in skill building.

Offer crisis management.

Collaborate with other community providers.

• Assist individuals in navigating complex systems.

Work with service recipients' informal supports.

Other duties as assigned by supervisor.
