Description : We are seeking a Physical Therapist responsible for the evaluation, planning, direction, and administration of physical therapy modalities of treatment as prescribed by a licensed physician. He / she administers treatments to restore function and prevent disability following injury or disease. He / she assists patients in reaching their maximum performance and level of functioning while learning to live within their capabilities. The Physical Therapist should demonstrate strong working knowledge and skill experience with various diagnostic groups, including orthopedic and arthritis-related conditions, neurological groups, and generalized weakness from medical or surgical conditions. A working knowledge of normal growth and development, needs, and appropriate treatment for the age and diagnosis of patient services is required for providing individualized care. Knowledge of and ability to apply IRF-PAI and Care Tool in the payment system for CMS is necessary. The Physical Therapist must administer evaluation tools per established protocols and utilize non-standardized probes for further assessment as needed. They should be able to evaluate patients effectively and establish an appropriate plan of care, adapting it to the individualized needs and goals of the patient and caregiver. Assessing patients’ pain using approved measurement tools and adjusting treatment accordingly is essential. The Physical Therapist should provide the proper assistance and cueing necessary for optimal performance and patient outcomes. They must effectively manage their therapy schedule, delegating appropriate portions of treatment to trained support staff as necessary. Performing home assessments and communicating results to patients and caregivers is also part of the role. The therapist must demonstrate the ability to conduct gait training, mobility instruction, and equipment use as needed. Knowledge of patient management with maladaptive behaviors is advantageous. Short and long-term goals must be established that are comprehensive, realistic, measurable, and involve the patient and / or family. Documentation of patient records must be complete, detailing function levels, treatment methods, patient responses, and progress with measurable goals. Recommendations for referrals to other professionals or agencies must be made as indicated by the plan of care. Patient and family education must be documented clearly, with home exercise programs provided as necessary. Effective communication with the patient and family regarding the role of Physical Therapy and treatment expectations is essential. Participation in weekly team conferences and collaboration with multidisciplinary team members to support patient care is expected. The Physical Therapist may assist with training and coordination of staff efforts with assistants and students. Planning and prioritizing the assigned caseload efficiently is a requirement of this position. Maintaining current knowledge of professional practice through continuing education and in-services is necessary for program improvement. Other duties may be assigned as required.
