The McClendon Center, founded in 1980, is a certified community based service provider operating under the District of Columbia Department of Behavioral Health. As a leading not-for-profit organization in Washington, DC, the McClendon Center provides outpatient behavioral rehabilitation to the mentally ill by fostering creativity, promoting friendship, instilling independence, and providing an outlet for emotional growth and greater participation in the community. Accredited by Joint Commission on Accreditation of Healthcare Organizations (JCAHO), the McClendon Center operates two programs : Day Program Services and the Core Services Agency (CSA).

McClendon Center operates programs in two locations, a day program near Metro Center and an outpatient service clinic in the NOMA section of DC. The Center has revenues of approximately $4.5 million, with contracts from the Department of Behavioral Health, three Managed Medicaid Organizations, and grants from various foundations.

McClendon Center is a respected and highly rated agency in the District of Columbia, having received the highest rating on the Department of Behavioral Health's provider scorecard three out of the last five years. The Center is also the only independent mental health agency in the District to be accredited by the Joint Commission on Accreditation of Healthcare Organizations.

Job Description

BASIC FUNCTIONS :

This is a hands-on, client-focused role requiring the incumbent to coordinate with various individuals, CSA teams, and outside agencies to ensure efficiency and a smooth transition throughout the agency intake process, and actively support clients through the continuum of care, including the discharge process.

Additionally, this position must be able to complete high-quality intakes and seamless transitions into the CSA / Day Program teams and participate in the prior authorization process for clinical services. The Intake Coordinator will work under general supervision of a licensed mental health professional.

PRINCIPAL ACCOUNTABILITIES :

  • Complete enrollment and data entry of clients into the electronic medical record and other programs upon being notified of a client's engagement.
  • Conduct a psychosocial intake, entering and scanning notes / documents into the electronic medical record as required within same day of service.
  • Travel within the community to complete intakes with clients when needed.
  • Obtain signed release forms and other required documentation from clients and coordinate appropriate follow-up care with the client's outpatient providers.
  • Ensure that all transitioning members are able to schedule a Comprehensive Psychiatric appointment and a nursing assessment within 21 days or less upon intake.
  • Complete required outreach within listed timeframe for consumers who have missed intake appointment.
  • Manage intake schedule effectively to ensure that productivity requirements are met on a daily basis.
  • Enter any diagnostic information obtained from past service providers into the client's medical record, attaching any required notes and supporting documentation regarding the diagnosis.
  • Complete Diagnostic Assessments (DAs) as required for clients' annual requirements.
  • Liaison with clients' outpatient providers, communicating all relevant information relating to the client.
  • Create a treatment plan for each transitioning member within day of enrollment.
  • Complete service notes day of service to include all outreach attempts.
  • Record prescribed medication into clients' medical record, as required.
  • Meet with Clinical Supervisor for regular supervision, as required, and update on client's status post-enrollment.
  • Make referrals to the Center's Day Program for clients who need daily structure and rehabilitation.
  • Complete all required clinical and administrative documentation for client discharges.
  • Actively participate in the prior authorization process and submits authorizations through Comagine for services as needed.
  • May perform other duties as assigned.

Qualifications

SPECIALIZED KNOWLEDGE AND SKILLS REQUIREMENTS :

  • Ability to coordinate with a multi-disciplinary team within an outpatient mental health agency.
  • Knowledge of sound ethical clinical practice and principles related to the care of mental health consumers.
  • A superior communicator, who shares information readily and concisely, listens as well as gives advice, and respects the abilities of others.
  • Excellent written and verbal communication skills.
  • Excellent computer skills, to include operating Microsoft Office Suite programs.
  • Knowledge and ability to navigate an Electronic Medical Record (EMR), preferably Credible.
  • Ability to relate to adults diagnosed with mental illnesses, using knowledge of concepts, principles, and theories related to mental health and mental illness.
  • Ability to perform amicably in an environment serving the mentally ill adult population.
  • Ability to respect and uphold client confidentiality.
  • Ability to exhibit a high level of organizational skills and able to prioritize and multi-task without direct supervision
  • MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS :

  • Licensed in the District of Columbia as a LICSW, LPC, LGPC, or LGSW is required.
  • At least two (2) years of experience working with individuals who have mental health diagnoses.
  • Able to coordinate with a multi-disciplinary team.
  • Physical Demands : The position requires moderate physical effort on a daily basis such as walking, bending, stooping and standing for periods of time.
