Medical Director (Family Medicine or Internal Medicine or Geriatrics) – Post Acute Care – Albany, NY

Opportunity Highlights

Leadership role overseeing post-acute care clinicians in the Albany area
Monday–Friday schedule with no night shifts or weekends
Conduct clinical case reviews and support care coordination initiatives
Collaborate with pharmacists and care teams to improve patient outcomes
Focus on quality, readmission reduction, and individualized patient care
$25,000 sign-on bonus

Candidate Requirements

Board Certified in Internal Medicine, Family Medicine, or related field

Prior experience as Medical Director in a skilled nursing facility preferred

Active New York license and DEA

Experience in post-acute, hospice, home health, or hospital medicine a plus

Compensation & Benefits

Estimated annual earnings : $310,000 to $345,000 (fee-for-service model, uncapped)
