RN Case Manager, Home Care

Registered Nurse (RN) Case Manager - Home Care : Enjoy independence, autonomy, and flexibility in the field to visit patients one on one in their home environment. The RN Case Manager would provide head to toe assessments, care coordination, disease and medication management, skilled wound care, catheter management, ostomy education, infection prevention, and home safety.

Pay and Benefits :

• Annual Salary Range : $89,419 - $106,620

Hours : Full-time | Days | Monday - Friday | Holiday rotation (work approximately two holidays per year) | Weekend rotation may apply dependent upon staffing levels.

Territory : This position will work in the field helping patients in their home setting. The office is in Roseville, MN and the general territory for this position of where the patients live would be the South East section of the Twin Cities Metro area.
