Salary : $100,000 - $115,000 per year

Job Details

Hiring a Full-time, Hospice RN Case Manager with active New Jersey RN license in good standing, who resides within or in close proximity to Passaic and Morris County NJ.

MUST have at least 2 years RN experience working in Hospice / Palliative Care OR home Health to be considered.

As a member of the IDT, the RN Case Manager works under the general direction of the Director of Nursing, the attending physician and the interdisciplinary group in providing skilled nursing assessments, planning, and care in order to maximize the comfort and health of both patients and their loved ones / caregivers.
