Make a real impact on patient care as a Pharmacist specializing in Medication Therapy Management (MTM) with a leading healthcare provider working with Aquent. You will play a crucial role in improving patient health outcomes by providing comprehensive medication reviews and consultations. This is an opportunity to leverage your clinical expertise to make a tangible difference in patients’ lives.
As a Pharmacist focused on MTM, you’ll be responsible for conducting comprehensive medication reviews (CMRs) and providing in-depth consultations on statin medication use, adhering to American Heart Association (AHA) guidelines. You will engage with patients through outbound calls, gathering and transcribing essential prescription and diagnosis information into a secure web-based application. Your meticulous documentation will contribute to the creation of personalized patient documents. This role requires a high volume of patient interaction, with an expected 100 outbound calls per day.

The target hiring compensation range for this role is $53.87 to $59.86 an hour. Compensation is based on several factors including, but not limited to education, relevant work experience, relevant certifications, and location.
