• This is a 100% field-based position covering the Round Rock Metroplex and surrounding counties.
• You must currently live in the Round Rock area.
• Relocation candidates and remote applicants will NOT be considered.
• Daily in-person visits to hospitals, skilled nursing facilities, PCPs, and senior living communities are required — this is not remote or hybrid.
• You must already have established relationships with discharge planners, case managers, hospitalists, or referring physicians in the Houston market.
