In this role, you will :

• Provide assessments, brief therapy, and crisis intervention
• Create goal-oriented treatment plans and track progress
• Collaborate with medical providers, teachers, and families
• Support patients in chronic disease self-management
• Offer behavioral health education to staff and caregivers
• Contribute to a strong, integrated care model
