Join our dedicated team at Ovation Hospice as a compassionate and skilled Registered Nurse - Case Manager. We are seeking a talented professional who is passionate about providing exceptional care in geriatrics, hospice, and palliative settings. In this vital role, you will plan, coordinate, and deliver hospice care aligned with each patient’s unique plan of care, while upholding the highest standards of care for our patients.

Responsibilities :

Provide nursing services, treatments, and preventative procedures as ordered by the physician, utilizing your specialized skills.

Understand and assess different levels of care appropriate for patients.

Apply the nursing process effectively to deliver high-quality care.

Continuously evaluate the effectiveness of hospice services for both the patient and their family.

Conduct evaluations for hospice care and provide informational visits as required.

Collaborate with interdisciplinary teams to develop and refine patient-specific care plans.

Encourage a culture of teamwork, respect, and collaboration among nursing staff and interdisciplinary team members.

Engage in community outreach, educational programs, and professional development activities.

Work alongside other department leaders to enhance organizational effectiveness and efficiency.

Qualifications :

Current Registered Nurse (RN) license in good standing.

Bachelor's degree in Nursing.

Minimum of 1 year of RN experience; hospice experience is preferred.

Demonstrated clinical assessment and critical thinking skills.

Strong organizational and problem-solving capabilities.

Excellent communication and interpersonal skills, enabling you to build rapport with patients, families, and team members.

Proficient in electronic medical record (EMR) systems and healthcare software.

Compassionate, empathetic, and committed to delivering high-quality end-of-life care.
