Your Role as a Hospice Registered Nurse :
Serve as the primary contact for patients and families, guiding them through their entire hospice journey with care and understanding.
Deliver and document skilled nursing care tailored to each patient's individualized Plan of Care, whether in their home, assisted living, or another chosen setting.
Conduct initial and ongoing assessments to monitor patient conditions, adjusting care as necessary to ensure optimal comfort.
Collaborate with a multidisciplinary team, including physicians, LPNs, CNAs, social workers, and chaplains, to develop and refine personalized care plans.
Offer emotional, physical, and spiritual support to both patients and their families, providing education, comfort, and bereavement guidance.
Educate caregivers on safe and confident care practices, ensuring they feel equipped to support their loved ones.
Share vital updates on patient conditions with the care team to facilitate timely and appropriate interventions.
Maintain accurate, up-to-date clinical documentation while adhering to infection control and compliance standards.
Contribute to quality improvement initiatives and scheduling efforts to ensure consistent, compassionate, and efficient care delivery.
Assist patients and families in understanding key topics including medication administration, hospice philosophy and services, symptom and pain management, and end-of-life processes and expectations.
About You - Qualifications :
Active RN license in your state of practice (or eligibility to obtain it).
At least 1 year of RN experience in hospice, home health, ICU, oncology, geriatrics, or similar nursing fields.
A genuine compassion for hospice care : empathetic, patient-centered, and dedicated.
Comfortable providing care in a variety of environments, including private homes and healthcare facilities.
Strong communication and critical thinking skills in emotionally sensitive situations.
Valid driver's license, auto insurance, and reliable transportation.
CPR certification is required.
Preferred Experience (but not Required) :
Hospital RN experience (Med-Surg, ICU, ER, PACU, telemetry, cardiac).
Experience as an Admissions RN, in wound care, float pool, or travel nursing.
Experience supporting terminally ill patients or serious illness care.
