Speech Language Pathologist (SLP) – Home Health

Schedule :  Monday–Friday, 8 : 00 AM – 5 : 00 PM

Job Type :  Full-time or Part-time (DOE)

Position Summary

We are seeking a  Speech Language Pathologist (SLP)  to join our growing home health team. In this role, you will provide skilled speech therapy services to patients in their homes, helping them regain and improve communication and swallowing abilities.

Key Responsibilities

  • Deliver care in accordance with state speech therapy practice acts, the patient’s plan of care, and in coordination with the interdisciplinary team.
  • Evaluate patients’ functional abilities using appropriate tests and assessments.
  • Develop and implement treatment plans to address communication and swallowing disorders.
  • Document visit notes and patient responses during or immediately after each session.
  • Educate patients, families, and caregivers on therapy techniques they can use at home.
  • Collaborate with supervising RN and other team members to adjust care plans as needed.
  • Participate in staff meetings, in-services, and case conferences.
  • Maintain up-to-date records and communicate changes in patient status.
  • Complete evaluations and care plans in accordance with state guidelines.
  • Provide occasional in-services upon request.

    • Qualifications

  • Current Oregon license in Speech Language Pathology (required)
  • Current CPR certification (required)
  • Valid driver’s license, proof of insurance, and reliable transportation (required)
